GP Information


When referring a patient to St Hugh’s Hospital please outline to them that the time from the referral by you to treatment at St Hugh’s will be no more than 18 weeks. Your patient must be prepared to attend outpatient appointments, diagnostic tests and admission to hospital for treatment within this time.

If your patient is going away for three or more weeks and their condition is not urgent, please consider referring them on their return. Patients that make themselves unavailable within the timeframes or do not attend their appointments will be discharged back to your care.

Please ensure the following information is included when referring your patient:

Full practice information including full postal address and surgery practice code

Patient’s full name and contact information (including daytime and evening contact numbers)

NHS number

4

Diagnostic test results (if necessary)

5

Relevant medical history


Some patients may be unsuitable for surgery under general anaesthesia at St Hugh’s. It is expected that the decision about unsuitability for operation at St Hugh’s is taken early.

Note: If the patient is referred for a service or treatment that is not routinely offered by the NHS then the process is different. Please submit an Individual Funding Request (IFR) to your CCG and provide details of where the patient wants to go. CCG’s will publish information about Individual Funding Requests on their website.
